Early versions of satellite and radiosonde datasets suggested that the tropical surface had warmed more than the troposphere, while climate models consistently showed tropospheric amplification of surface warming in response to human-caused increases in greenhouse gases. We revisit such comparisons here using new observational estimates of surface and tropospheric temperature changes. We find that there is no longer a serious discrepancy between modeled and observed trends in the tropics. Our results contradict a recent claim that all simulated temperature trends in (...) the tropical troposphere are inconsistent with observations. This claim was based on the use of older radiosonde and satellite datasets and on two methodological errors: the neglect of observational trend uncertainties introduced by interannual climate variability and application of an inappropriate statistical “consistency test”.This emerging reconciliation of models and observations has two primary explanations. First, because of changes in the treatment of buoy and satellite information, new surface temperature datasets yield slightly reduced tropical warming relative to earlier versions. Second, recently developed satellite and radiosonde datasets now show larger warming of the tropical lower troposphere. In the case of a new satellite dataset from remote sensing systems, enhancedRSS warming is due to an improved procedure of adjusting for inter-satellite biases. When the RSS-derived tropospheric temperature trend is compared with four different observed estimates of surface temperature change, the surface warming is invariably amplified in the tropical troposphere, consistent with model results. Even if we use data from a second satellite dataset with smaller tropospheric warming than in remote sensing systems RSS, observed tropical lapse rates are not significantly different from those in all model simulations.Our results contradict a recent claim that all simulated temperature trends in the tropical troposphere and in tropical lapse rates are inconsistent with observations. For over a decade, managed care has profoundly altered how healthcare is delivered in the United States. There have been concerns that the patient-physician relationship may be undermined by various aspects of managed care, such as restrictions on physician choice, productivity requirements that limit the time physicians may spend with patients, and the use of compensation formulas that reward physicians for healthcare dollars not spent. We have previously published data on the effects of managed care on the physician-patient relationship from (...) the physician's perspective. In 1999, we collected data on the impact of managed care arrangements on the physician-patient relationship from the patient's perspective. This article discusses our collective findings. (shrink)

Over the past several years, healthcare has been profoundly altered by the growth of managed care. Because managed care integrates the financing and delivery of healthcare services, it dramatically alters the roles and relationships among providers, payers, and patients. While analysis of this change has focused on whether and how managed care can control costs, an increasingly important concern among healthcare providers and recipients is the impact of managed care on the physicianpatient relationship, but little data have been collected and (...) analyzed. We designed a survey for distribution to Wisconsin physicians to analyze the prevalence and types of managed care arrangements in the state, and the impact of these arrangements on physicians and their relationships with patients. Neural engineering technologies such as implanted deep brain stimulators and brain-computer interfaces represent exciting and potentially transformative tools for improving human health and well-being. Yet their current use and future prospects raise a variety of ethical and philosophical concerns. Devices that alter brain function invite us to think deeply about a range of ethical concerns—identity, normality, authority, responsibility, privacy, and justice. If a device is stimulating my brain while I decide upon an action, am I still the author of the (...) action? Does a device make the interiority of my experience accessible to others? Will the device change the way I think of myself and others think of me? Such fundamental questions arise even when a device is designed for only a relatively circumscribed purpose, such as restoring functioning via a smart prosthetic. We are part of a National Science Foundation-funded Engineering Research Center tasked with investigating philosophical and social implications of neural engineering research and technologies. Neural devices already in clinical use, such as deep brain stimulators for Parkinson's disease or essential tremor, have spurred healthy debate about such implications. Devices currently under development—such as the BrainGate System of implanted brain sensors coupled to robotics in persons with paralysis, exoskeletons for augmented movement, transcranial do-it-yourself stimulators, closed-loop brain stimulating systems, or even brain-to-brain interfacing—promise to extend and deepen these debates. At our center, brain-computer interfaces are the principal focus of work. Even acknowledging that the clinical translation of neural devices and seamless integration by end users may still largely reside in the future, the potential these devices hold calls for careful early analysis. The launching of the Brain Research through Advancing Innovative Neurotechnologies Initiative in April 2013 provides further impetus for this work. (shrink)

Drug companies are moving their research from academic medical centers to physicians’ private offices. The shift brings in more subjects, and could mean faster and better results. It also changes the physician's relationship to patients, dangles monetary lures in front of physicians, and could produce subjects who don't understand what they're participating in and results that are unreliable.

In an effort to assess the latest thinking in the Roman Catholic Church on economic matters, we examine the newest encyclical by Pope Benedict XVI, Caritas in Veritate (Charity in Truth) for guidance concerning marketing and business strategy. Core ethical values, consistent with historical Catholic Social Teachings (CST), are retained. However, some important nuances are added to previous treatments, and, reflecting the mind of the current Pontiff, certain points of emphasis are shifted to account for recent global developments. Key areas (...) of consistency and differences (as we perceive them) are spelled out along with some brief commentary on the evolution of the CST position on matters of importance to business decision makers. We close our analysis with a brief discussion of how the lessons of the encyclical can be applied to selected marketing problems embedded with ethical issues, including some criteria for evaluating marketing programs. Finally, we note some editorial commentary published in the wake of the letter’s release along with our own summation. (shrink)

In proposition 7 of the second part of the Ethics, Spinoza famously contends that the “order and connection of things is the same as the order and connection of ideas.” On this basis, Spinoza argues in the scholium that thought and extension are different ways of conceiving one and the same substance: “the thinking substance and the extended substance are one and the same substance, which is now comprehended under this attribute, now under that”. Less famously, in the same scholium, (...) Spinoza insists that the relation of thought and extension is not the only issue at stake. He notes that “certain Hebrews” saw, “as if through a cloud,” the unity of God and the world in knowing: “they maintained that God, God’s intellect, and the things understood by him are one and the same.” The cloud is the idea of divine transcendence. Commenting on this passage, Aviezer Ravitzky has observed that Spinoza’s mention of these “certain Hebrews” should inform us about “an inherent problem, about dynamite concealed in the teachings of the Jewish Aristotelians such as Maimonides.” If, as Aristotle argues in the De Anima and Metaphysics, the knower and the known are one in knowing, then the distinction between God and the objects of God’s knowing is undermined. Noetic union is ontological union, for the intellect is its ideas. Thus Aristotelian noesis undermines divine transcendence. E2p7 and its scholium, then, presage the most famous—or infamous—expression of the Ethics, Deus sive Natura. Maimonides qualifies his Aristotelianism with an hierarchical emanationist cosmology, thereby avoiding the implications of noetic union by differentiating between the creator and the creatures and between the agent intellect, the lowest of the celestial spheres and source of the human acquired intellect, and the divine intellect. Gersonides, the fourteenth century giant of the medieval Jewish tradition and its most consistent Aristotelian, endorses the Aristotelian formula, arguing consistently that the intelligible is an intellect and offering a meticulous explanation of “why it is said of non-material things that the intellect, the thinker, and the intelligible are all one.” In this article, I argue that Gersonides’ own highly Averroian position is evoked in E2p7s and that Spinoza extends and radicalizes the Gersonidean inheritance. Beginning from E2p7 and its scholium, I explore the implications of noetic union for the relationship of God and the world and that between thought and extension. (shrink)

"Dreaming with open eyes" is a tagline for Spinoza's critique of Descartes; the dreams in question are principally those of volition and the active imagination. In this article, I compare the Cartesian theory of imagination as an active, but not fully rational, power of the mind and the Cartesian account of the volitional self to Spinoza's views. Descartes's own dreams and theories of dreaming are the focus of the first part of the article. Thereafter I examine Spinoza's critique of Descartes (...) and his alternative account of imagination. Finally, I argue that there is a positive sense of dreaming with open eyes to be recuperated in Spinoza's thought. Construed positively, to dream with open eyes is to understand dreams and imagination as natural phenomena and so to be able to respond constructively to them in ethical and political, as well as epistemological, life. Spinoza is often conceived as a highly intellectualist philosopher, and it is tempting to read human freedom without attention to its material basis. In this paper, I study Spinoza's claim that the more the body can undergo, the more the mind can know in order to establish Spinoza's view of freedom under the attribute of extension.

In Ethics 4, Spinoza argues that “A free man thinks of nothing less than of death, and his wisdom is a meditation on life, not on death” (E4p67). Spinoza’s argument for this claim depends on his view of imagination, reason, and scientia intuitiva and on his notion of conatus. I explicate Spinoza’s view of life in terms of power (potentia) and show that Spinozan death amounts to reconfiguration rather than absolute annihilation. I then show that E4p67 reflects Spinoza’s well-known account (...) of the three kinds of knowing. Thinking of death is quintessentially imaginative and passive. The free person of E4p67 is in contrast a rational person. To reason, and as becomes especially clear E5, to experience scientia intuitiva, moreover is to think of things “without any relation to time, but [rather] sub specie aeternatitis”(E2p44c2) and to experience activity To the extent, then, that we are rational, free, and active, death is a non-issue. Indeed, to the extent that we are able to meditate on life sub specie aeternitatis, we actually experience joy, love (E5p20s, p32c), eternity (Sp23s), and “the greatest satisfaction of the Mind” (E5p27). Our goal was to explore self‐care practices among men who have sex with men in the context of Mpox in Brazil. This study used qualitative research methods, including interviews and thematic analysis, to collect and analyze data from male participants across the Brazilian territory. The narratives unveil men's perspectives on self‐care, risk reduction, and health beliefs during the Mpox pandemic. Our findings highlight a multifaceted approach to self‐care among men, encompassing hygiene, physical contact management, mask usage, skin lesion vigilance, and (...) adherence to official guidelines. Men's attitudes toward sexual behaviors emphasize the importance of reducing sexual partners, practicing safe sex, and combating misinformation through accurate information dissemination. The development of these behaviors and self‐care practices can be facilitated by nurses guided by Dorothea Orem's Self‐Care Theory, supported by patient‐centered care, with strategies to address and confront the stigma associated with the disease and provide emotional support. Thus, the study underscores the pivotal role of self‐care in mitigating infection risks, especially in the context of emerging infectious diseases. It acknowledges the impact of socio‐cultural factors and healthcare policies on men's preventive measures. However, it also recognizes limitations, such as potential bias due to stigma concerns and a nonrepresentative sample. Ultimately, the research advocates for tailored education, promotion of gender equity, and healthcare empowerment to effectively manage health risks in such contexts. (shrink)

Academic freedom has become the enemy of the individual professors working in colleges and universities across the United States. Despite its historical (and maybe even essential) roots in the First Amendment, contemporary case law has consistently shown that professors, unlike most members of society, have no rights to free speech on their respective campuses. (Ironically, this is especially true on our State campuses.) Outlined is the dramatic change in the history of the courts from recognizing “academic freedom” as a construct (...) needed to protect professors from the status quo, to the abuse of “academic freedom” appropriated to protect the institution from “undesirable” professorial actions such as politically incorrect speech or research. Klein warns all those in the academy to become familiar with this pernicious 180-degree turn in the use of the “academic freedom” construct. (shrink)

Considered in its seventeenth-century context, Spinoza’s way of thinking about substance and nature is striking for its simultaneous refusal of Cartesian dualism and Hobbesian materialism. Spinoza knew both thinkers’ work well, yet sided with neither. Where Descartes divides substance into thinking and extended substance, and where Hobbes reduces all things to body, Spinoza espouses what is best called a double-aspect or non-reductive monism. The single substance of the Ethics is expressed as an infinity of modes in an infinity of attributes, (...) each infinite in kind. Thought and extension are two of the infinite attributes of substance, and the simultaneous sameness and difference of thought and extension constitutes a major theme of the Ethics. Using exactly parallel demonstrations, Spinoza argues in the second part of the Ethics that substance, or God, or nature is equally a thinking thing and an extended thing. Descartes’ account of matter and extension, in which passive matter is set in motion by God, inspires harsh criticism from Spinoza. (shrink)

Through a close analysis of texts from the Second Objections and Replies to the Meditations, this article addresses the tension between the pursuit of certainty and the preservation of divine transcendence in Descartes’s philosophy. Via a hypothetical “atheist geometer,” the Objectors charge Descartes with pantheism. While the Objectors’ motivations are not clear, the objection raises provocative questions about the relation of the divine and the human mind and about the being of created or dependent entities inDescartes’s metaphysics. Descartes contends that (...) there are real, eternal essences present in the human intellect as innate ideas. I argue that this claim implicates him in pantheism, not merely univocity. In the course of the analysis, I consider recent interpretations by Wells, Marion, and Hatfield. (shrink)

It is meant to inspire more thought and debate on just what sport is, how it relates to other activities and human endeavors, and what we can learn about ourselves by studying sport.

There has been much discussion concerning the consequences of 'going natural', i.e., of replacing a priori epistemology with empirical psychology. Traditionalists claim that a naturalized epistemology is not viable—to eliminate the normative from an account of knowledge is to cease to do epistemology at all. Naturalists claim that a naturalized account is the only viable one—assuming, in step with the urgings of Quine, that there are no standards independent of (and external to) science, science itself must act as the sole (...) epistemic norm. In the wake of the above debate some epistemologists have attempted to argue in favor of, and develop, a middle-ground position— normative naturalism . Such a position is intended to be consistent with the naturalist's intuition that the traditional search for a 'first philosophy'is misguided and consistent with the traditionalist's intuition that a complete elimination of the normative would leave epistemology impotent. In this paper I will examine one argument in favor of a normative naturalism . / will show that such a proposal is inherently problematic and argue that naturalism and normativity are mutually exclusive concepts. Therefore, I suggest that, even in the 'age of cognitive science', epistemologists continue to do traditional epistemology by attempting to develop, a priori , the general criteria for determining the justificatory status of our (scientific) beliefs. This article discusses the impact of Descartes’s substance-dualism on his account of discursive reason. Taking the presentation of deduction in the Rules as a paradigmatic case of thought’s extension and movement in time, I analyze the relation between intuitive and discursive understanding and that between intellect and imagination. I focus specifically on the mediation of corporeal impressions and of intellectual ideas by ingenium. As intellectual, ingenium is a faculty of understanding; as joining with phantasia, ingenium has access to corporeal affections, (...) images, and memory. Deduction involves both of these aspects of ingenium, and Descartes’s dualism complicates efforts to clarify the operations and nature of ingenium. Thus the dynamics of dualistic psychology account for some of the limitations of deduction in particular and discursive rationality in general. (shrink)

Terrorism, international gangs, and other frequently mentioned national security threats are actually less dangerous than a new type of state-corporate crime that may be called national insecurity crime. This crime poses not only unprecedented victimization, but a massive ethical problem. Examples in the U.S. include the 1980s Savings and Loan (S&L) scandal, the late-1990s dot-com bubble, the 2003 U.S. invasion of Iraq, and the 2007–09 financial crisis. National insecurity crime threatens national security because of its geographic and social extensiveness, severity (...) of harm, dynamism, and unpredictability. It endangers the political, economic, social, and environmental security of most or all of the U.S. population. This endangerment includes increasing the risks of loss, suffering, and destruction on a large scale. This article offers a starting point for a sociological criminology of national insecurity crime. National insecurity crime involves large-scale cooperation that relies on often indirect or mediated interaction between perpetrating persons, groups, and organizations. It also involves widespread social harm for private gain, and yet, due to ideology and social construction, it tends to be hidden in plain sight. (shrink)
